چکیده

Methods We have observed 10 consecutive patients with flail chest at 1,5-year period. They were 7 male, 3 female. The age was 42-71 years. Four patients also had multiple injuries and underwent operations on other organs. The terms before thoracic operation were 24 – 72 hours. Number of fractured ribs was from 4 to 10. The fractures of right ribs were at 3 patients, of left ribs – at 2 patient, bilateral rib fractures had 5 patients. Hemopneumothorax was observed at 5 patients, lung laceration – at 2 patients, fracture of sternum – at 2 patient, fracture of clavicle – at 1 patient, rupture of left cupula of diaphragm – at 1 patient. All patients had different degree of respiratory insufficiency, 3 of them had ARDS.

Crossed Kirschner’s wires for the treatment of anterior flail chest: an extracortical rib fixation

Objective: Thoracic trauma may be a life-threatening condition. Flail chest is a severe chest injury with high mortality rates. Surgery is not frequently performed and, in Literature, data are controversial. The authors report their experience in the treatment of flail chest by an extracortical internal-external stabilization technique with Kirshner’s wires (K-wires). <...

New insights into the pathophysiology of flail segment: the implications of anterior serratus muscle in parietal failure.

OBJECTIVE The wisdom of surgery facing multiple and multi-focal ribs fractures (flail segment) remains controversial. By the present retrospective study, we sought to determine the advisability of surgery as well as the anatomical and biomechanical features of flail segment leading to secondary dislocation. METHOD From 1970 to 2000, 127 patients underwent flail segment osteosynthesis. Clinica...

The modified Ravitch approach for the management of severe anterior flail chest with bilateral sternochondral dislocations: a case report

The management of flail chest continues to evolve as scientific evidence and surgical experience accumulates. Flail chest injuries that span the sternum present a rare and complicated injury pattern that can be challenging to manage both medically and surgically. Our patient is a 69-year-old involved in a high-speed motor vehicle crash with respiratory failure secondary to an anterior flail che...

Rib Fractures: To Fix or Not to Fix? An Evidence-Based Algorithm

Rib fractures are a common injury resulting from blunt chest trauma. The most important complications associated with rib fractures include death, pneumonia, and the need for mechanical ventilation. The development of new osteosynthesis materials has stimulated increased interest in the surgical treatment of rib fractures. Surgical stabilisation, however, is not needed for every patient with ri...

Surgical stabilization of flail chest injuries with MatrixRIB implants: a prospective observational study.

BACKGROUND Surgical stabilization of flail chest injury with generic osteosynthesis implants remains challenging. A novel implant system comprising anatomic rib plates and intramedullary splints may improve surgical stabilization of flail chest injuries. This observational study evaluated our early clinical experience with this novel implant system to document if it can simplify the surgical pr...
